Annual Young General Assembly Member Organization Report Form

The Annual Report Form is important to the Young General Assembly. It provides necessary information so a profile of the organization can compiled for requests for funding, media inquiries, and descriptive information. Each Member Organization is required to turn in a fully completed Report Form at the end of each year in order to keep its Member Organization status with the Young General Assembly. Failure to do so by the due date results in dismissal.

1.     At the top of the page place the full name and current address of the Member Organization with current telephone, fax, e

mail addresses ( and web site address if available).

2. The purpose of the organization is___________________________(fill in a one or two sentence statement )

3.     The Member Organization has _______people under 18 and ______ people over 18. (Fill in numbers)

4. The Member Organization operates on a local___, partially national____, fully national___ or international

scale___. ( Place an x after the appropriate words)

(e.g. members, foundations, local government, etc.)

6. Describe any formal relationship the Member Organization has with any United Nations agency______

8. State the “Action Area” of any Young General Assembly designed projects from the “Let’s Do it!” books from
